Position Information: The Supv, Community Case Management works collaboratively with the Community Case Management leadership to carry out department goals. Ensures consistent standards of practice. Ensures exceptional community case management staff competencies, and appropriate staffing to meet patient and family needs. Recruits and orients behavioral health, substance abuse and case management staff, and performs their evaluation. Serves as clinical resource person to the staff, to problem solve difficult patient related issues and coaches staff related to best professional practice. Assists with the development of Community Case Management services by identifying issues and implementing new department initiatives. Works closely with physicians, nursing, members of the interdisciplinary team, patient financial services, patient access and other departments, to ensure that services are being provided in a timely/quality manner, to ensure coordination between departments and to improve resource utilization. Utilizes data to assist with the analysis of the effectiveness of the Community Case Management.
Experience Requirements: 3 Years Clinical - Case Management Or Clinical - Discharge Planning Required - And 1 Year Supervisory Required
Education Required: Bachelor's Degree Related Field Required - Or Master's Degree Related Field Required
Licensure/Certification Requirements: Registered Nurse Or Licensed Clinical Social Worker Or Licensed Professional Counselor Or Licensed Clinical Addictions Specialist Or Certified Case Manager Preferred
