The Physical Therapist (PT) is a member of the Physical Medicine and Rehabilitation Service (PM&RS) staff. The incumbent is responsible for providing evaluations, as well as planning and administration/management of rehabilitation services for patients enrolled in or referred to Physical Therapy at the Memphis, TN VA Medical Center. The Physical Therapist (PT) works under the general supervision of the Chief of PM&R through the Section Chief, PT, who sets the overall objectives to be met and resources available. The Physical Therapist (PT) performs appropriate cleansing and re-processing of reusable medical equipment in compliance with VHA directives and Standard Operating Procedures. The PT keeps up-to-date with current trends in rehabilitation medicine/therapy through scheduled in-services, library resources, continuing education courses and hospital training and development workshops scheduled by the facility. The functions of the position include, but may not be limited to the following: Completes other administrative duties as assigned (work orders, FSOD, information processing, program development, etc.) for the PM&R Service or Medical Center. The PT independently completes a review /examination of patient data (including medical history) to develop a plan of care, assess patient's status, and ascertain patient's need for therapy services. Plans and facilitates treatment programs for patients with complex impairments and providing therapeutic interventions to meet functional goals. Attends staff meetings as directed. Maintains timely and appropriate documentation of all consults, initial evaluations, progress notes, discharge notes, patient/family education, work load statistics, QA monitors, patient attendance records and other clinical activities. Inputs scheduling data into VISTA computer system or through approved charting methods and updates daily to reflect current status of patients. Completes all employee/patient incident documentation in a timely manner as required by hospital or Service policy. Performs non-technical maintenance of equipment to prevent malfunctions. Provides guidance and support to professional and technical students. Performs comprehensive evaluations directed at ascertaining the patient's functional status, rehabilitation needs, goals, and rehabilitation potential. Establishes therapeutic agendas that are age-appropriate and which are based on the patient's identified deficits and expressed goals. Performs evaluations to include medical history, results, and interpretation of appropriate functional tests and measurements such as range of motion, muscle strength, sensory/perceptual deficits, coordination, functional mobility, pain scales, and social needs. Provides therapeutic interventions, which focus on posture, locomotion, strength, endurance, balance, coordination, joint mobility and flexibility. Modifies treatment plan according to patient response to best meet established goals. Documents treatment progression and/or modifications according to departmental and Medical Center standards. Ensures appropriate patient referrals and discharge planning in concert with the physical therapy and Medical Center staff.
