The Acute Care Nurse Practitioner (ACNP) manages patients on their specialty service/practice under the direction and indirect supervison of theri supervising physician, who may or may not be the service line medical director. DEPARTMENT SPECIFIC DUTIES: Each ACNP will have a broad spectrum of duties, relating to patient care, and may also have very specific duties related to their specific serviceline. Generic duties include: â�¢ Attend daily rounds with the physician as needed. Develop a treatment plan based upon direction and guidance provided by the physician on daily rounds. â�¢ Collect database information by performing a physical assessment and constructing a problem list. â�¢ Monitor and evaluate patientâ��s hospital course, participating in patient diagnosis, management, evaluation, and plan. Order and evaluate daily lab tests, radiographs, and studies. Initiate written orders and consults based on the treatment plan. â�¢ Assess and intervene in the general health care needs of specialty patients to facilitate efficient treatment of and effective recovery from the medical/surgical problem. â�¢ Promote health by providing preventive health education and intervention appropriate to patientâ��s age and medical/surgical problem and/or risk. â�¢ Provide teaching and support to patients and families related to their specific medical problem and treatment and generate referrals to community resources as needed. Ascertain patient and family understanding of medical/surgical problem and prognosis. Provide detailed information to patients and their families regarding surgical/medical diagnosis, expected course and prognosis, and plan of care. â�¢ Evaluate complex cases and establish a plan of care for rehabilitation. Facilitate and make referrals for support services that may include but are not limited to Occupational Therapy, Speech Therapy, Physical Therapy and Physical Medicine and Rehabilitation. â�¢ Monitor and evaluate patients for discharge. Evaluate and define discharge regimen, complete discharge exam, write discharge orders, dictate comprehensive summaries for co-signature, recommend follow-up and consult referrals. â�¢ As appropriate, ACNP may function in the outpatient clinic setting. Functions include: o Independently provide episodic and hospital follow-up visits. o Monitor on-going patient problems, providing counseling and coordinating of resources. Monitor for resolution of oncology/medical/surgical problems after hospital discharge. o Evaluate new patient complaints and complete work-up as indicated, including review of radiographs, labs and studies. o Perform standardized procedure functions. â�¢ Provide telephone advice and referrals to patients. â�¢ Complete SVRMC and clinic records, including but not limited to progress notes, orders, radiograph requests, and study requests. Dictate records when appropriate. â�¢ Perform extended role procedures as defined in standardized protocols and standardized procedures, including but not limited to the RNFA standardized procedure. â�¢ Act as a resource for staff and other hospital personnel. Serves as an expert consultant in their specialty area. Participates in ongoing educational events. â�¢ Participate in specialty specific research grants and studies. â�¢ Maintains current professional licenses and/or certifications as and when required by state, regulatory agency, or hospital mandate Surgical: Nurse Navigator Nurse Practictioner will build on a legacy of nursing excellence by caring with compassion and advocating for the health of patients, families, and communities. The Nurse Navigator primary function is to serve as first contact for patients to assist with logistical navigation through the perioperative care continuum; provide a link between patients and physicians; act as the liaison between clinical specialists and physicians to improve quality, service, outcomes, and coordination of treatment. Increases access to care and guides patients and families through the health care system from the time of diagnosis through their duration of care by assisting them with access to needed services, developing relationships with various providers, increasing throughput efficiencies, perioperative goals and metrics, and tracking interventions and outcomes. Coordinates this care to ensure safe, effective, efficient patient centered care.
Experience with interdisciplinary health care approach and coordination/managment of the surgical patient experience Strong analytic skills with quality and data analysis. Required: MSN, 2-5 years related work experience in Surgical Services
Required: CA RN License, BLS, ACLS, PALS, ACNP
Required: Computer skills to include: EMR (Cerner), Microsoft Office and Web Based Applications Preferred: Nurse Navigator experience Preferred: Certification . Preferred: Two years of clinical experience as a nurse practitioner.
